1. Data Controller

Premier Vision Eye Care is registered with the Data Protection Commission (DPC) of Ghana as a Data Controller. We are committed to safeguarding the privacy and personal health information (PHI) of our patients in strict compliance with the **Data Protection Act, 2012 (Act 843)**.

2. Information We Process

Through our digital appointment request form, we process the following patient data:

• Full Name (for patient registration and record matching)
• Ghana Mobile Telephone Number (to confirm and coordinate scheduling)
• Email Address (optional, for digital confirmations)
• Selected Eye Care Service (to allocate clinical resources)
• Preferred Appointment Date

3. Purpose and Legal Basis

In accordance with Act 843 Principle 2 (Consent), we process your contact information solely to coordinate and schedule your eye care appointment. Processing is based on your explicit, active opt-in consent given via the booking form. We do not use your contact information for unsolicited marketing or sell it to third parties.

4. Zero-Retention Transmission Policy

To enforce maximum data minimization:

• Our online form transmits data securely using end-to-end TLS 1.3 encryption.
• The relay processor is configured with a zero-retention policy, immediately deleting your submission data from intermediate database servers once forwarded to our secure email inbox.
• We store your records locally within our physical clinic file system under strict medical confidentiality protocols.

5. Patient Rights

Under Act 843, you maintain the following rights regarding your data:

• The right to request access to your recorded personal data.
• The right to rectify inaccurate or incomplete records.
• The right to withdraw consent and request the deletion of your personal data.
• The right to lodge complaints with the Data Protection Commission of Ghana.
